Prepayment is Required

We require prepayment based on the records review that is required and the complexity of the questions you would like answered.  Call us for ACH / payment details.

Exams are generally scheduled 4-6 weeks in advance.

If the claimant has x-rays, MRIs or other images that are older than 6 months, we require a fresh set of x-rays on CD prior to arrival.  

If the case involves carpal tunnel or tarsal tunnel or neuropathy, a fresh EMG /NCV is required prior to arrival.

We are one of the few practices in the state of Utah that uses the AMA 6th edition calculations for TTD, PPD, and MMI reports. 

We are also one of very few orthopaedic specialists in Utah that accepts these cases. 

Turnaround for transcription is usually 7-10 days from the exam.

  1. Call our office manager (435) 523-3378 and request the appointment. 
  2. Our in-house healthcare paralegal will take down certain information and provide the quote.  
  3. Have number of pages count ready, SOAF, questions, and any other pertinent information ready for the call.
  4. You’ll make the payment, forward the records as documents, (not scans of pictures of documents) and confirm the date and time with the claimant. 
  5. No show fee/late arrival fee is $150.  Please advise any claimants traveling from Las Vegas of the time change!
